PATIENTS

To request a copy of your medical records, please CLICK HERE. 

There may be a reasonable, cost-based fee associated with your medical records request pursuant to federal regulation 45 CFR 164.524(c)(4).

PROVIDERS

If you are a physician, physician's office, or other healthcare provider requesting medical records for a patient in your care, please fax a request on your letterhead with the patient's name, date of birth, and information needed to (405) 486-2266.
